Initial Comments: The purpose of the inspection was to conduct a complaint investigation for complaint #166893 on 04/22/2026. A full inspection was not conducted at this time. The Notice of Inspection Rights were provided to the Licensee at the time of the Inspection. Ratios observed were: Infants: 1:4 1-yr-old Children: 2:10 2-yr-old Children: 1:8 2-yr-old Children: 2:12 3-yr-old Children: 1:13 3-yr-old Children: 2:16 4-yr-old Children: 2:21 4-yr-old Children: 2:22 There were 3 Staff interviewed during this investigation. Others interviewed: Director, Assistant Director Documentation observed was: One staff file, Child Attendance records, Child Information, Information, and Immunization Record cards. Upon completion of the complaint investigation, it was determined from observations, interviews, and documentation that the 1:3 allegations was able to be substantiated. The following deficiencies were observed and are subject to changes pending programmatic review. -Patting of child’s back during nap time. -Tone of voice while re-directing children.

Initial Comments: The following deficiencies were observed at the time of the Initial inspection conducted on 9/29/2025, and are subject to changes pending programmatic review. 25 staff files were reviewed, and all fingerprint clearance cards were verified as valid via the DPS website. A paper copy of the Notice of Inspection Rights was provided to the Licensee during the inspection. The following was discussed, but not limited to: * Registration of Empower and completion of Emergency Disaster Preparedness forms following initial licensure, * Completing all staff file requirements following initial licensure, * Ensuring playground equipment is maintained in a clean condition, * Ensuring toy containers and buckets are maintained in a clean condition, * Ensuring seats in the facility's motor vehicle are maintained free from hazards, * Ensuring staff use appropriate discipline and guidance methods. Please submit the following: * Written plan of corrections via the licensing portal, * Photos/documentation of corrections. NOTE: Licensure is pending the above requirements.

Please don’t take ur children here , my 3yr old non verbal on the spectrum child was slapped in the face by a teacher , the other teacher witnessed it and tried to tell the director Andrea which she told her keep the incident between them, nom call or incident report was given to me , I asked to see the cameras which they sd nothing was recorded but only live video , violence against children at this preschool has to b stoped , thers always issues wit this school B AWARE
