Noncompliance Area: During the renewal inspection, the emergency contacts for Child #2 and Child #3 did not accompany the classroom staff while on a walking excursion to the community playground.

Correction Required: When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion.

Noncompliance Area: During the renewal inspection, the immunization record for Child #1 did not include a flu shot. A statement of exemption from the child's parents was not on file.

Correction Required: Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Noncompliance Area: During the renewal inspection, the posted certificate of compliance had expired on 7/1/19. The current certificate of compliance was not posted.

Correction Required: The facility's current certificate of compliance and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location.

Noncompliance Area: During the renewal inspection, the file of Staff #1, hired 10/1/18, Staff #2, hired 6/18/18, Staff #3, hired 11/12/18, did not contain a signed employee disclosure statement required under the CPSL.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #1, #2 and #3 may not work in a child care position at the facility.

Noncompliance Area: During the renewal inspection, the files for Staff #1, hired 10/1/18, Staff #2, hired 6/18/18, Staff #3, hired 11/12/18, did not contain documentation of training regarding the emergency plan at the time of initial employment.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: During the renewal inspection, the file for Staff #2, hired 6/18/18, did not contain verification of child care experience, education and training, for position performing, following outset of service at the facility.

Correction Required: An aide shall have attained one of the qualification levels specified in 3270.37(b)(1)-(3).A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: During the inspection it was determined that chemicals, in a closet, were stored above gloves, plates and paper towels.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.
