Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Additionally, on 5/1/24, Staff #2 was observed in the PK classroom unsupervised by a staff person at minimum as an AGS.

Correction Required: An aide shall have attained one of the qualification levels specified in §3270.37(b)(1)-(3). An aide or a combination of aides shall be supervised at all times by a staff person qualified at minimum as an assistant group supervisor.

Description: Mixed Age Level/Staff assigned to specific children

Noncompliance Area: On 4/18/24, Staff #3 left their assigned group of 7 PS children on the outdoor grassy area at the back of the facility unsupervised by a Staff person. Staff #3 left Individual #1 the responsibility of the 7 PS children. Individual #1 is not a qualified staff member. During this time, a jump rope was wrapped around Child #1 neck, leaving rope burns. Additionally, on 5/1/24 at time of inspection, Staff #4 was responsible for 4 children (3 infants/1YT) and Staff #5 and #6 were responsible for 8 Young Toddlers in the infant/toddler childcare space. Staff #4 left the childcare space for approx. 1-2 minutes to answer the door leaving Staff #5 and #6 out of ratio with 12 children (the youngest children were infants).

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times.T. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
