| Initial Comments: The following deficiencies were observed at the time of the Initial Monitoring Inspection conducted on 11/24/2025, and are subject to changes pending programmatic review. The Notice of Inspection Rights were provided to the Licensee at the time of the inspection. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. | Initial Comments: The purpose of the inspection was to conduct a complaint #149220 investigation on 10/30/2025. A full inspection was not conducted at this time. Ratios observed were: Infants: 2:7 1-year-old children: 2:8, 1:5 1-2-year-old children: 2:9 2-3-year-old children: 2:13 3-year-old children: 1:8, 2:18 4-year-old children: 1:13 4-5-year-old children: 1:9 There were 2 staff members interviewed during this investigation. Others interviewed: Owner and Facility Manager. Documentation observed: Emergency Record Cards, Staff Schedules, Attendance Records The fingerprint clearance cards for 2 of 2 staff members were verified to be valid at the time of the inspection. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 3 of 3 allegations were unable to be substantiated due to a lack of sufficient evidence. There were no deficiencies observed at the time of inspection. | |||
