|Findings: Specialist observed no evidence of a medical evaluation for one employee. Facility must submit a copy of the staff persons medical report to the office of child care.|
|Findings: Specialist observed no evidence of fire drills conducted June through October of 2018. Facility must submit a statement confirming that fire drills will be conducted monthly, moving forward.|
|Findings: Specialist observed no evidence of a thick gauze pad and liquid fragrance free soap with the first aid supplies. Facility must submit photographic evidence that missing items have been added to the first aid kit.|

|Findings: Specialist observed emergency forms missing required information and updates. Facility must submit a statement confirming that all children's emergency forms have been completed and updated, as required.|
|Findings: Specialist observed no evidence of a written screen time policy. Facility must submit a copy of the written screen time policy to the office of child care.|
|Findings: Specialist observed peeling paint in room 10. Facility must have area of wall with peeling paint repainted.|
|Findings: Specialist observed no evidence of a substitute log. Facility must provide a written statement confirming that a written substitute record will be used, moving forward, anytime a substitute is used in the place of the assigned staff in a child care room.|
|Findings: Specialist observed incomplete health assessments. Facility must submit a statement confirming that all children's health assessments have been fully completed, as required.|
|Findings: The complaint alleged on April 12, 2019, the director was seen dragging a child by their wrist down the hallway. The complaint alleged that the director on several occasions has yelled at students and grabbed them by the wrist and yanked them by the arm. Staff reported that they did not visually witness the incident on April 12, 2019, but some heard yelling at an inappropriate level. It was confirmed that on another occasion, a child was grabbed and pulled by the arm forcefully, due to the child's behavior; and at times, the Director does yell or speak in a way that can be intimidating. Facility must orient all staff to the facility's written discipline policy.|
