Noncompliance Area: In the nap room, it was observed that staff person #1 and #2 were unable to see the children in the room. Lighting in the room was insufficient and staff were using flashlights to identify the children.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: Upon entry into the nap room, staff were asked to identify the children they were responsible for at that time. Staff person #1 and #2 stated "they were responsible for all of the children." Staff person #1 named 3 older toddlers and staff person #2 named 1 younger toddler and 2 older toddlers, leaving 4 younger toddlers, 1 older toddler, and 1 preschool child unnamed. All children were napping at the time.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: In the main room upstairs, it was observed that there were 2 cracked magnetic tiles, exposing the children to pinch points.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In the drop-in daycare, main room, it was observed that mat in the infant section was torn on the corners exposing the children to foam. In the bathroom/changing area it was also observed that the changing pad was torn along the edge causing foam to be exposed that could have a diameter of less than 1 inch.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The lunch, homework, and music room upstairs, lacked a written plan of daily activities.

Correction Required: The written plan of daily activities shall be posted in the group space.

Noncompliance Area: In the turquoise room, at approximately 9:15 am, 3 staff were present with 18 preschool children, staff person #1 named 5 children, staff person #2 named 6 children, and staff person #3 named 6 children, leaving 1 child unaccounted for. In the drop-in main room, 4 staff were present with 15 children, staff person #1 named 4 older toddlers, staff person #2 named 6 preschoolers, staff person #3 named 1 preschooler, and staff person #4 named 2 preschoolers, leaving 2 children unaccounted for.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: The file for child #3 lacked a health insurance policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: In the homework room, the emergency transportation plan was not posted.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: The file for child #4 last reviewed the emergency contact and financial agreement on 4/26/19 and 1/8/2020, which is more that 6 months apart.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The file for child #1 and #2 lacked an influenza immunization record in accordance with the ACIP

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: There was no written consent for the Epinephrine injection for child #5.

Correction Required: A parent shall provide written consent for administration of medication or a special diet.

Noncompliance Area: The file for staff person #1, with a start date of 3/11/19, did not have a health assessment until 4/9/19. The file for staff person #2, with a start date of 6/24/19, did not have a health assessment until 10/1/19. The file for staff person #5, with a start date of 8/22/19, did not have a health assessment until 11/14/19.The file for staff person #9 had a health assessment dated for 10/18/16 and 1/9/2020, which is more than 24 months apart.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The file for staff person #1, with a start date of 3/11/19, did not have a tuberculosis screening until 4/9/19. The file for staff person #2, with a start date of 6/24/19, did not have a tuberculosis screening until 10/1/19.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: It was observed that 6 bottles were not labeled in refrigerator.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: Staff person #6, with a hire date of 9/8/15, had a health assessment with tuberculin skin test dated for 9/1/19, with no prior health assessments on file.

Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.
