Conditions at the facility may not pose a threat to the health or safety of the children. Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Then at approximately 9:50 AM it was observed that Staff person #2 was in the downstairs gross motor room with 11 preschool children. When interviewed Staff person #2 named and identified all 11 children as being in their designated group. Certification rep pointed out that they named 11 preschoolers and Staff person #2 quickly identified child #4 that was supposed to be in Staff person #1's group upstairs. Staff person #1 stated that child #4 must have gone downstairs when another child was using the restroom. Child #4 was not under supervision of any staff person when child #4 left the upstairs preschool room and walked downstairs to the gross motor room.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: At approximately 9:45 AM it was observed that Staff person #1 was in the upstairs preschool room with 10 preschool children. When interviewed Staff person #1 named and identified all 10 children as being in their designated group. When interviewed Staff person #2 named and identified all 11 children as being in their designated group. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

When interviewed Staff person #2 named and identified all 11 children as being in their designated group. Certification rep pointed out that they named 11 preschoolers and Staff person #2 quickly identified child #4 that was supposed to be in Staff person #1's group upstairs. Staff person #1 stated that child #4 must have gone downstairs when another child was using the restroom. Child #4 was not under supervision of any staff person when child #4 left the upstairs preschool room and walked downstairs to the gross motor room.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.

Noncompliance Area: It was observed in the basement gross motor space that in the corner by the restroom a piece of baseboard trim is loose. The radiator cover in the basement gross motor space and in the infant room were loose and have exposed screws. It was observed that the push bar on the door exiting the facility was loose and had pinch points. It was observed in the basement hallway leading out to the outside exit that there was unidentified black substance on the wall.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: It was observed that there was chipping and peeling paint around the side exit door. There was a sign on the exit ramp that had peeling paint that was accessible to the children. There was a shelving unit at the front entrance that has peeling paint.

Noncompliance Area: At the time of inspection, the facility lacked a proof of purchase for one of the smoke detectors installed and currently in use at the facility.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. §  1016(c)).The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.

Noncompliance Area: In the main child care room, observed a child's round table. All of the legs of the table were not tightly secured. This caused the table to be unsteady.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed that all of the newly purchased blue rest equipment at the facility lacked a label with the children's names.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Observed an infant sleeping in a swing for approximately ten minutes and not flat on his/her back as is the sleeping position recommended by the American Academy of Pediatrics. Medical documentation permitting the child to sleep in the swing was not present in the file.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: The files for children #1 and #2 lack a child service report within the previous six months and were due in the file prior to 3/6/2020.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: There was more than six months in between the previous and the most current and updated health assessment on file for child #2, who is a young toddler.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddlerA child's record shall contain initial and subsequent health reports.

Noncompliance Area: Observed three infant pack n plays. The sheets which were attached to the mattress pad of the pack n plays were very loose causing a strangulation hazard to the infants in care .

Noncompliance Area: The emergency plan was not posted in a conspicuous location. It was recently taken out of facility and forgot to bring back.

Correction Required: The operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: The file for staff #1, who was previously a volunteer at the facility and became a staff person on 8/23/2019, lacked the State Police and Child Abuse clearances required under the CPSL for a staff person. Previous State Police and Child Abuse clearances on file were for volunteer purposes. There is now a correct State Police clearance on file and dated 1/16/2020 and a correct Child Abuse clearance on file and dated 1/23/2020. The file for staff #2, with start date of 10/1/2017, lacked a National Sex Offender Registry certificate by 7/1/2020. There is no NSOR on file. The file for staff #5, with start date of 1/29/2020, lacked a National Sex Offender Registry certificate on or before initial start date in a child care position .There is now an NSOR on file and dated 2/7/2020.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #2 -- may not work in a child care position at the facility. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.
