UPON REVIEWING THE VIDEO ON 11/8/17 WITH THE PARENTS OF CHILD #1, THE STAFF PERSON WAS TERMINATED IMMEDIATELY. A POSITIVE GUIDANCE TRAINING WAS SCHEDULED FOR ALL STAFF ON 11/24/17. A COPY OF THE SIGN IN SHEET FROM THE TRAINING WILL BE KEPT ON FILE AT THE FACILITY. STAFF MEETING WAS CONDUCTED DISCUSSING SITUATION WITH STAFF. STAFF MAY NOT USE ANY FORM OF PHYSICAL PUNISHMENT IN ORDER TO REDIRECT A CHILD.
