Description: Child-Parent Centers was founded in 1967 in Tucson Arizona. We provide Head Start services to children and families in Pima, Cochise, Graham, Greenlee, and Santa Cruz counties. Head start is a comprehensive federal program administered by the Office of Head Start, within the Administration of Children and Families of the Department of Health and Human Services. Head Start is a federal program that promotes the school readiness of children ages birth to 5 from low-income families by enhancing their cognitive, social and emotional development. We work with parents and see them as their child's first and most important teacher.

Findings: * Surveyor 1 observed that Child 4 and 5 were wearing diapers in Classroom 3; which is not licensed for diapering. According to the Director, both children are special needs. The Director stated that both children are taken to Classroom 2; which is licensed for diapering when their diaper needs to be changed. Both children were recently enrolled, and the facility has not obtained an individualized plan for either of the children. However, Surveyor 1 reviewed each of their emergency records; which does not indicate that either child is delayed in their toileting and no special instructions were provided.

Findings: * Surveyor 1 used a Department approved tool to measure the resilient surface. The base of the yellow slide on the Eastside of the playground measured 3", the base of the yellow slide on the Westside measured 4" and the tire swing measured 3" of the required 6" of a resilient surface.
