Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 4/9/2025 and completed on 4/10/2025, and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***The "Playpod" structure in the outdoor activity area is approved for children ages 5 - 12 only. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 7 of 7 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Field trip requirements. 2. Licensed activity areas. 3. Licensing the Atrium. 4. Individualized plans. 5. The first aid kit must contain sterile gauze rolls. 6. Supervision of children in the toilet rooms. 7. Cleanliness of toilet room vents. 8. Flooring must be sanitizable. 9. Nicked toilets seats must be repaired or replaced.

Initial Comments: The following deficiencies was found at the time of the Compliance Inspection conducted on 4/16/2024 and is subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 3 staff files reviewed. 3of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.
