|Findings: Surveyor 1 observed the sign in and out records and found Child 9 and 10 were present, however not signed in for 10/23/2018.|

|Findings: Surveyor 2 reviewed 2 staff files. Neither Staff 1(DOH 1/5/12) nor Staff 2 (DOH 1/5/12) had completed 18 annual training hours as required for 2017-2018 year.|
|Findings: Survyeor 1 observed urine in the toilet in the preschool children's bathroom.|
|Findings: Surveyor 1 observed the emergency exit in the preschool room did not have a building evacuation plan by the door leading to the playground.|
