Noncompliance Area: File for Staff # 1 did not contain a health assessment. Staff began employment on 8/25/18.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Fee Agreement for Child # 1 has not been reviewed and/or updated by parents since 2/5/18.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: File for Staff # 1 did not contain a valid FBI Clearance. File contain a paper from the Dept. of Ed, stating the unofficial results are enclosed. Staff began child care on 8/25/18.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Infant bottles were not labeled with child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: Pack n' play were not labeled for each child.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Infant bottles were not labeled with each child's name.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: Files for Staff # 1, # 2 and # 3 did not contain verification of child care experience.

Correction Required: A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: Health assessment for Child # 1 did not include a review of child's immunization record.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: Facility hot water that is accessible to children was measured at 118 degrees.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: There is one crib at the facility that does not have documentation on file to verify that it meets the current safety standards as required by the CPSC

Correction Required: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement.

Noncompliance Area: During the inspection on 4/21/17 there are eight chidren present that include three older toddlers and five young toddlers. Facility person #1 stated that she and facility person #2 were watching the children as one group. The children are not specifically assigned to a staff person.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Child file #1 does not have a fee agreement on file

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: Child files #2 and 3 are missing the addresses of the release persons on the emergency contact forms

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.
