Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Surveyor observed 2 activity areas (of 2 areas) and found that the areas were adhering to group size requirements at the time. The sand on the outdoor area was observed to be wet and frozen in the fall zones of the climbing equipment and was unable to be evaluated for resilient surface depth. | |||
