|Findings: a) Team Leader reviewed twelve children's emergency records. The following information was missing: b) Child 4 - the back side of the original emergency record was missing. c) Child 6 - the parent answered "yes" to all the questions on back of the emergency record, but no other information was provided.|
|Findings: a) Team Leader reviewed twelve children's emergency records. b) Children 1-5 - the copies of the emergency records that are taken out in an emergency were missing the back side of the emergency records.|
|Findings: a) Surveyor reviewed the roster in the one-year-old classroom. There were 5 children present, but only 4 documented on the roster.|
|Findings: a) Team Leader reviewed the personnel file for Staff # 4 who was listed as the Director Designee. Upon review of her file, Team Leader observed that she only has 22 of the required 30 training hours.|
|Findings: a) Surveyor observed Staff 5 and 6 alone in the one and two-year-old classrooms. Upon review of their personnel files Team Leader observed that Staff 5 did not have any documented child care experience. Staff 6 had an on-line diploma. Upon further research of the diploma, Team Leader verified through their website that the diploma could be obtained by taking a four week course and one test, which is not equivalent to a GED or high school diploma.|
|Findings: a) Team Leader reviewed three personnel files. The files for Staff #5 did not have documentation of their good faith references.|

|Findings: a) Surveyor observed that the infant room was not equipped with an adult size chair.|

|Findings: a) Surveyor observed a mat being utilized on top of the diapering surface in the infant and two-year-old classrooms which are not sanitizable.|
|Findings: a) Team Leader measured the water at the diapering sinks to be only 52 F.|
|Findings: a) According to the staff in the one-year-old classroom, they only use bleach and water to sanitize the diapering surface. They do not use soap and water to clean it.|
