| Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. A small Christmas tree was was plugged into an electrical outlet. The electrical cord for the tree had an exposed electrical outlet on the outer surface of the cord. When measured, there were areas that measured two (2) inches and there were areas under the slides that measured less than one (1) inch.
