Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. A school -aged child was crying and trying to hot another child who took their snack from them and the staff were unaware.

Violation Observed: Ensure that adequate staff/child ratios are maintained at all times throughout the center. Observation revealed three children who are 3years old, three children who are 4 years old and 55 children who are ages 5- 13. This mixed age group requires 5 staff; whereas 3 staff were present with children and the director was in and out of the rooms for various reasons. The center must ensure adequate staff/child ratios at all times.

The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.

Violation Observed: Ensure that adequate procedures for tracking children are adhered to at all times throughout the center. When transitioning from the classroom to the playground, staff were unaware of how many children were in their care. One staff stated that 46 children were present; however, after counting 3 times, the staff confirmed that 43 children were going out to the playground.

In the transition from school to the program, there was one staff member was with 25 children. Other staff were present and were walking in and out of the room, thereby leaving the classroom out of ratio.

Disinfectants shall be used as follows: the center shall disinfect those items specified in N.J.A.C. 3A:52-7.7(a)3 with a solution that shall be a commercially prepared disinfectant that indicates it kills bacteria, viruses, and parasites. This solution shall be used in accordance with label instructions.

Violation Observed: Ensure that the center uses a commercial disinfectant in accordance with the manufacturer's instructions. The product that is being used needs to remain on the surface for 4 minutes to be effective and that is not the practice at the site.

Handwashing requirements are as follows: the center shall ensure that children three months of age and older wash their hands with soap and running water before intake of food.

Violation Observed: Ensure that children wash their hands before the Immediate intake of food. Ensure that hand washing takes place immediately prior to eating. Observation revealed children sitting on the floor after hand washing and before eating.

The center shall develop and follow a policy on the administration of medication and health care procedures to children, which shall include the following provisions: the center shall store prescribed epinephrine pens in their original boxes with the child’s name and prescription. A special care plan or other documentation from a health care provider shall accompany the epinephrine pen. If two epinephrine pens are required, both shall be available. Each center under the jurisdiction of N.J.S.A. 18A:40-12.5 shall store back-up epinephrine pens on site consistent with the requirements stated at N.J.S.A. 18A:40-12.5.e(1).

Violation Observed: Ensure that all Epi-pens are stored in their original boxes with the child's name and prescription, accompanied by a special care plan, and that two are present when required. Ensure to follow care plan.
