Noncompliance Area: A cracked plastic toy bin, approximately 4"x12" was observed in an area accessible to children in care in the Toddler Room. The edge of the cracked plastic posed a laceration threat.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Staff #11, who was responsible for four children in her supervisory group, was unable to identify any of the children by name.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: The files for Child #1 and #4, both of whom have been in care for more than six months, did not include child service reports.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: The file for Child #3 included an emergency contact form that did not include the name of the child's health insurance provider, nor did it include the health insurance policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The file for Child #2, who is two years and five months old, included an immunization record; however, said record has only two HIB vaccinations recorded. The third dosage is recommended at he age of six months for the four dosage vaccine or between 12-15 months for the three dosage vaccine. If the child is being administered the four dosage vaccine, the immunization record is missing two vaccines.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: The file for Child #3 includes a health assessment on which the Physician checked no to the question regarding the child's ability to participate in child care and the child appearing to be free from contagious disease.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Noncompliance Area: The menu posted near the entrance to the facility was end-dated 05/17/2019.

Correction Required: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family.

Noncompliance Area: In the infant room, an approximately 4' pedestal fan was observed sitting atop an approximately 18" high storage bin. Fan was accessible to children in care and could be easily tipped which could cause a trauma injury.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The files for Staff #1-#10 did not contain documentation of the completion of emergency plan training. The files for Staff #1 and Staff #10 did not include documentation of the completion of Mandated Reporter training.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: The files for Staff #3, #4, #6, #7, #8, #9, and #10, all employed for more than 12 consecutive months, did not contain documentation of the completion of Fire Safety training.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: The file for Staff #6 contained a Child Abuse Clearance that was completed more than five years ago.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #6 may not work in a child care position at the facility.

Noncompliance Area: The files for Staff #2, #3, #6, #8, #9, and #10, all of whom being employed for more than 12 consecutive months, did not include documentation of annual staff evaluations

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: A surge protector in the Toddler room, in an area accessible to children under the age of six, was observed to have two unused, unprotected outlets.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: The water temperature used by children in the Pre-school room was measured at 115 degrees Fahrenheit.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: A metal locking mechanism located on a storage cupboard located in the Toddler Room, in an area accessible to children in care, was observed to have sharp edges that could cause laceration injuries. Also in the toddler room, an approximately 2"x2" broken piece of plastic (possibly used to mount intercom or phone) was observed to be mounted to the wall in an area accessible to children in care, which also could cause laceration injuries.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
