Findings: The Team Leader reviewed the personnel file for Staff 1 and observed that she was missing documentation of at least 30 hours of training. The Provider acknowledged that Staff 1 is listed as the person to act on her behalf when she is not present in the group home.

Findings: The Provider acknowledged that she did not have a lock box for medications that need to be refrigerated.

Findings: The Surveyor observed that the high chair located in the dining room did not have the safety straps attached to it. The Provider said that she had recently washed the straps and was unable to find them during the inspection. The Surveyor received a photograph of the chair with the straps attached to it within a few hours of the inspection.

Findings: The Surveyor observed that there was a training potty chair sitting on the bathroom floor. The Provider was advised that the potty chair is not plumbed into the wall with water pressure and therefore is not allowed to be used.
