Findings: The Surveyor observed that a fire drill was logged on a Sunday, 2/10/19, when the center is not in operation.

Findings: The Team Leader and Surveyor reviewed that statement of services and found the following: 1. The hours of operation stated weekend hours and the center is not licensed for weekend care. 2. The statement did not have the field trip procedures. 3. The medication procedures said life threatening medications would be locked, and staff stated that they keep the medication in the kitchen out of reach but not locked.

Findings: The Surveyor reviewed the postings and found that the name of the Director, the name of the individual to act as a designee, and the fees and refund policy was not visible. Staff found the postings behind the fire drill log.

Findings: The Surveyor observed staff leave 2 children unsupervised at the tables by the kitchen, when the staff went by the diapering area with another child.

Findings: The Surveyor observed the fire extinguishers and found the following: 1. The classroom fire extinguisher did not have a year punched on the tag, only the month. 2. The kitchen fire extinguisher was expired as of March 2018.

Findings: Based on direct observation and discussion with the staff, the Surveyor observed the following: a. There were seven enrolled children present when the Department arrived at the facility. There were only five listed on the roster. b. According to the staff two children had just arrived; based on sign in and out records the children arrived approximately fifteen minutes prior to the Department arriving.

Findings: Based on a review of two staff files, the Team Leader observed the following: a. Staff #1 had four hours of training between 3/3/17-3/3/18. b. Staff #2 had zero hours of training between 9/27/16-9/27/17. c. According to staff, the hours are on the Central Registry, which the Department does not have access to.
