Findings: Based on the Surveyors' observations, it was determined that the following hazards were accessible to enrolled children: 3's-4's Classroom: *There was a loose electrical cord on the floor near the dry erase board. *There were two large panels, (1 cardboard and 1 pegboard) leaning against the wall. *There was one tall mirror leaning against the wall. *There was a small TV on a shelf near the East wall that was unstable. *On the South side of the room, there was a metal plant holder on the stage, that was unstable and tipped easily. *On the stage, there was peeling tape. 2's Classroom: *There were two TV cords loose and accessible to the children. *The TV was unstable and could be tipped over easily. *There were three large sections of turf (used as carpets) with fraying threads on the edges. 1st Toilet Room: *There was cracked caulking between the wall and the back part of the sink. *Previously Cited: 08/06/2020.

When the Surveyors' were checking the restrooms and opened one of the restroom doors, a child was in the restroom alone and unsupervised.
