Findings: 5 CSR 25-500.182 Child Care Program Violation: The requirements for admitting and dismissing children were not met as evidenced by the caregiver dismissed a child to an unapproved individual. Rule Reference: 5 CSR 25-500.182 Child Care Program (1) (A) 2. states: A caregiver personally shall admit each child upon arrival and personally shall dismiss each child upon departure. Children shall be dismissed only to the parent(s), guardian, legal custodian, or to individuals approved by the parent(s), guardian, or legal custodian. Correction Required: Each child shall be admitted and dismissed by a caregiver as required. Correction Verification: Submit Documentation Compliance Date: 4/7/2025 5 CSR 25-500.212 Transportation and Field Trips Violation: Appropriate care, safety and/or supervision of children was not provided while transporting children as evidenced by On March 26, 2025, Staff Kara Little, Autumn Stonage and Brianna Veach and were outside putting children on the bus when a parent stepped in and told her they were going to the school and could take 2 school aged children, Child A and B, to the school for her. Staff Kara Little allowed the children to get in a car with the parent. (This parent was neither of the children's parent or relative). The parent did not have written permission on any enrollment forms to transport the children or pick the children up from the center. Kari Miller called the school immediately to check on the children and assure they were at the school. She then called the parents of both of the children and reported to them what had happened earlier that morning. A staff meeting has been scheduled for next week to review the rules for admitting and dismissing children at the center. The facility will send notes from the meeting and a signed statement from all the employees that they understand the licensing rules. The signed statement and meeting agenda will be sent to the Office Of Childhood. Staff Kara Little no longer works at the facility. Rule Reference: 5 CSR 25-500.212 Transportation and Field Trips (1) (A) states: The provider shall be responsible for the care, safety and supervision of children on field trips or at any time they transport children away from the facility.
