Noncompliance Area: All drills conducted since 4/2019 have been between 8:00am & 9:30am.

Correction Required: Fire drills shall be held at different times of the day or night, or both, if applicable.

Noncompliance Area: Rep observed a child identified as an infant sleeping in a pack & play with an infant gym toy and a pacifier with a stuffed animal on the end.

Correction Required: Toys, bumper pads, or pillows may not be present in a crib while an infant is sleeping in the crib.

Noncompliance Area: Child #, a 2 yr old, has only 2 doses of HepB vaccine, but according to ACIP recommendation should have 3 doses. Also child #1 does not a current influenza vaccine. The child does not have a proper exemption letter on file.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: Facility person #1 had emergency plan training on 8/26/17 and then again on 10/19/18, a span of over 1 year.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: Facility person #1, date of hire 6/18/18, does not have initial emergency plan training documented in the file. Facility person #1 has annual emergency plan training dated 8/23/18.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: Observed bleach solution on an accessible shelf in the K Readiness Room. Also observed small vials of oils in an accessible desk drawer in the Large room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: The small refrigerators in the K-Readiness room and the Older toddler room did not have thermometers in them. Food provided to the children are kept in both.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: Children #1-#4 do not have child service reports on file. Children have admission dates of 8/28/17, 8/21/17, 8/30/17 and 8/28/17, respectively. Operator reports that the facility is not currently using an approved child service report or assessment.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: Child #3 does not have addresses for all release persons listed on form.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Observed diaper cream in the men's room that was in an accessible unlocked drawer.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Noncompliance Area: The current certificate of compliance and the emergency plan are not posted.

Correction Required: The facility's current certificate of compliance and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location.The operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: Facility person #1 was hired as an AGS but does not have 2 years of experience with children verified on file.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children.

Noncompliance Area: Observed children receiving child care in K-Readiness and older toddler rooms. Operator acknowledged that school age children receive care in room called the school age room. None of these rooms appear on the list of measured child care space.

Correction Required: Indoor child care space is measured within permanent stationary partitions or walls. The allowable number of children in a space is determined by dividing the total square feet in a space by 40.

Noncompliance Area: Observed a missing outlet cover on an accessible outlet in the preschool room located on the raised portion of the room. Also observed an accessible power strip near the CD player in the older toddler room.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Observed a can of Lysol disinfecting spray that is labeled ''keep out of reach of children'' on an accessible shelf in the ladies room that children use.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: The 1st aid kit in the K-Readiness room next to the desk in an accessible area. The 1st aid kit in the school age room was hanging on a hook approximately 4 feet from the floor which is accessible to school age children.

Correction Required: A first-aid kit must be inaccessible to children.

Noncompliance Area: The fire drill log indicates that a fire drill was held on12/8/17 and not again until 3/5/18 which is longer than 60 days apart.

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.

Noncompliance Area: Facility persons #6, 7, & 8 are missing updated health assessements. Facility person #13 is missing an initial health assessment. Date of hire 9/16/16.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Facility person #13 does not have a TB result on file.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: The facility does not have a current copy of the facility's liability insurance, expired 1/1/17.

Correction Required: The legal entity shall have comprehensive general liability insurance to cover the persons who are on the premises. A current copy of the insurance policy shall be on file at the facility.

Noncompliance Area: Facility persons #1 & 13 are missing two written nonfamily references.

Correction Required: A facility person`s record shall include two written, nonfamily references from individuals attesting to the person`s suitability to serve as a facility person

Noncompliance Area: Facility persons #1,2,3,4,5,6,7,8,9,10,11,12,13,14 & 15 do not have proof of initial and annual emergency plan training.
