Noncompliance Area: On 3/14/25 the director reported that a child was left outside on the playground on 3/13/25. Director reported child was outside alone for approximately 5 minutes. The facility was made aware that the child was missing when a head start staff brought the child into the day care.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: 1. Children must be supervised at all times. This portion of this plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of in person supervision training. The legal entity must receive DHS approval of the training content prior to scheduling the training. The training cannot be conducted when children are in care. The operator shall provide a date for when this training will be completed. 3. The legal entity must create a supervision policy to include name to face recognition as well as a policy for supervision during transition periods. This policy must be approved by DHS before implementation. DHS will make no less than two unannounced visits over the next two months to ensure staff are complying with the supervision policy. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The correction date for this portion of the plan shall be at minimum two months from the date the Regional Office approves the policy.

-Staff will be utilizing lanyards with names and pictures for counting purpose. -Procedure change: extra person will help classes go out and re-enter the building. Children will be counted on porch before going up stairs and in the classroom. -All staff took training "outdoor supervision and counting" Children will be supervised at all times moving forward. The training for staff #1 is to be completed on the BetterKidCare website if it is approved by the state. Each staff member will also complete this training and will learn how to make sure that each child is always supervised while they are in their care. The training is called: Supervision of Children in Family Childcare, and it is a 2-hour long training course. This training teaches staff how to supervise children, how to keep them safe, and healthy. The supervision training is based on inside and outdoor spaces. Our policy on supervision will be in place for all staff to be trained in and for all new staff as well once it is reviewed by the state and can be put in place. The staff will keep each child safe by counting the children in their care before they leave their classroom, in each stairwell, before they head out the door to go outside, and again when they get on to the playground area. The staff will count every 10 minutes while outside to ensure the safety of the children. Staff will always position themselves inside and outside to be able to hear, see, direct, and assess each child while in their care throughout the day. When the teacher is ready to come back inside, they will follow the same routine of counting the children as they did when they came outside. The staff will count before leaving the playground area, at the door where they come in, at each stairwell, and once they get back into the classroom.

Our policy on supervision will be in place for all staff to be trained in and for all new staff as well once it is reviewed by the state and can be put in place. Children are to be appropriately supervised at all times in and outside of the building. At Little Desk Preschool we want to maintain a safe and secure environment for all the children. We will always maintain correct adult-to-children ratios. When going outside staff will count the number of children they have before heading down the stairwell. Staff will have their classroom clipboard with them with all their children's schedules in their hands as they are going downstairs. The staff will count and make sure that all the children in their care are accounted for once they get to the first stairwell. They will count at the second stairwell and again at the last stairwell before heading out the door. Before going out the door to the outside the staff will count again to make sure everyone is accounted for. Once the teacher is ready to go outside the teacher will hold anyone's hand that has to be held. If there is more than one child whose hand must be held then the teacher will be provided with a walking rope or will have an extra teacher to help them outside. Once the teacher gets to the end of the building the teacher will then stop and count again. If there are two staff members going outside with the same group, then one staff member will be in the front of the group while the other staff member is at the back of the group of children. Once they get to the playground the teacher will count the children as they enter the playground area. The teacher or teachers will position themselves so they can supervise all areas of the playground. The staff will spread themselves out to be able to see each child, hear each child, direct what each child is doing, and to assess or assist what each child is doing. The staff will count the children every 10 minutes while they are outside to ensure that all children are safe and accounted for. When the teacher is ready to come back inside, they will follow the same routine of counting the children as they did when they came outside. Here at Little Desk Preschool, we will make sure that all children are supervised at all times during the day while they are in our care. Staff are to carry their classroom clipboards with them outside. On their clipboards is a schedule of all the children who are in their care. The schedule contains the days and hours that each child is here for. On each day of the week there is a number of how many children are in that teachers care for that day. The staff are to count the children before they go outside at each stairwell, and once outside when they get to their destination, such as the playground. Once, in the playground area the staff will count the children every 10 minutes to ensure the safety of all the children in their care. If there are 2 staff outside with the same group, then they will be assigned to certain children who they are responsible for and for counting while outside. Once, going back inside the staff will count again to make sure that everyone is in the group while leaving the playground area. The staff will count once they get to the door to head inside, at the bottom of the stairwell, at the next stairwell, and lastly when they make it back to their classroom. If any child were to leave while they are outside, then it will be marked on that teacher's clipboard. The staff will also keep track of the number of children they have left in their care if a child does leave to go home while outside.

Staff #1 was immediately suspended on 3/8/23 and sent for the DHS FBI. Staff #1 is the legal entity and rarely is in the building and never in ratio or direct care with children. Staff #1 had a full PDE FBI on hand at the facility that indicated no issues and able to work with children.

Effective 2/24/22 staff #1 will be suspended until they obtain the DHS FBI instead of the DOH FBI. The staff will be reinstated once the completed DHS FBI is obtained. A note regarding the suspension will be placed in the staff file as well.
