Noncompliance Area: The files for children #1, #2, and #3 lacked reports which provide information to the family about the child's growth and development in the context of the services being provided. The reports were due prior to the regulation being suspended due to COVID 19.

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: The files for children #1, #2, and #3 contained immunization records which lacked documentation of receiving the influenza vaccine within the past 12 months.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: The file for staff#1 contained clearances which were not updated within 60 months as required. The previous state police clearance was obtained on 6/20/14 and was updated on 2/7/20. The previous child abuse clearance was obtained on 6/24/14 and was updated on 3/9/20. The previous FBI clearance was obtained on 6/18/14 and was updated on 3/25/20. All clearances on file for staff #1 are now current and valid.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: The file for staff #1, designated as an assistant group supervisor, did not contain a high school diploma.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children.A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: Four cribs in use by 2 infants and 2 young toddlers were not labeled for the use of a specific child.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The files for children #1 and #2 did not include verification of having received the influenza vaccine within the last 12 months.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The manufacturer's instructions for use of safety restraints was not in the vehicle used by the facility to transport children on the day of the inspection.

Correction Required: Manufacturers' instructions for use of safety restraints shall be kept in the vehicle at all times.

Noncompliance Area: The files for staff #1, #2, and #4 contained documentation indicating they did not participate in fire safety training at least annually. Staff received training on 2/18/2018 and again on 3/24/2019.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: The file for staff #1 contained an FBI clearance which was not renewed within 60 months. The initial FBI clearance was obtained on 10/16/2013 and was updated on 3/16/2019. The file for staff #2 contained an FBI clearance which was not updated with 60 months. The initial FBI clearance was obtained on 10/15/2013 and was updated on 3/16/2019. The file for staff #3 contained 3 clearances which were not renewed within 60 months. The initial state police clearance was obtained on 10/21/2013 and updated on 2/22/2019, the initial child abuse clearance was obtained on 10/26/2013 and updated on 3/3/2019, and the initial FBI clearance was obtained on 10/31/2013 and updated on 3/26/2019. The file for staff #4 contained 2 clearances which were not renewed within 60 months. The initial state police clearance was obtained on 10/21/2013 and updated on 2/22/2019. The initial child abuse clearance was obtained on 10/24/2013 and updated on 4/1/2019.

Noncompliance Area: In the infant room, a child was observed sleeping in a crib which lacked two feet of space on 3 sides. Access to the crib was blocked by another crib and a bouncy seat.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: The file for child #1, now school-age, contained a health report which has not been updated since the child was 3 years old.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.A child's record shall contain initial and subsequent health reports.

Noncompliance Area: The emergency plan lacked a specific plan for sheltering in place at the facility.

Correction Required: The facility shall have an emergency plan that provides for shelter of children during an emergency including shelter in place at the facility and shelter at locations away from the facility premises.

Noncompliance Area: Staff #1 applied for fbi clearance after start date. Staff #1 can not work in a position at the facility. Staff #2 completed mandated reporter training 107 days.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 2 may not work in a child care position at the facility.

Noncompliance Area: A reusable spray bottle which lacked a label specifying the contents was observed in the restroom off the play space. Based on statements made by staff, the bottle contained room deodorizer.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The glass door in the preschool/school-age room lacked visual identification.

Correction Required: A visual strip or other visual identification shall be placed on glass located in a traffic area, a child care space or a play space.
