Description: The licensee failed to ensure that all information provided to the department was current & accurate. Regarding lunch service, Staff B reported "we have a teacher that prepares the meals at her home." Per the licensee, Staff B prepared the center lunch at the licensee's family child care center on 7/27/21. Conflicting information was provided to the department as to whether or not any children were inside the center on 7/27/21 as well as the hours worked by Staff B on 7/27/21. The licensee provided conflicting information about whether or not children were served breakfast on 7/27/21.

Description: Per the report of the licensee and Staff B, center lunches have been prepared at the licensee's home since the 6/10/21 center visit. This meal preparation location has not been inspected by a representative of a state agency as required. The kitchen at the licensee's home was observed on 7/28/21 and was observed to be unsanitary and in a state of disarray. The center lunch was prepared at the licensee's home on 7/27/21 per the report of the licensee.

Description: There is not an adult-size rocking chair or other adult-size chair provided for each child care worker in the infant classroom. There are 3 children under the age of 2 in care and no chairs available in the classroom.

Description: A leftover bottle for a 10 month old infant is not discarded after use. After the infant is given the bottle it is placed in the refrigerator in the infant room and given to the infant again.
2021-07-27 251.09(2)(a) Infant & Toddler - Responding To Crying Children
Description: The licensee and Staff B failed to respond promptly to a crying infant's needs when a 10 month old infant is observed screaming and crying on and off for an extended period of time. For example, when the infant is crying after being placed in a crib the licensee is told by Staff A "it's OK..he can cry." A 5 month old infant also cries for an extended period of time while lying in a Pack N Play in the nap room.
2021-07-27 251.06(9)(g)1.c. Meal Preparation Staff - Handwashing
Description: Staff did not always wash their hands with soap and warm running water prior to handling food. For example, a staff person served pretzels to children by scooping them out of a bucket with his bare hands. He failed to wash his hands prior to handling the pretzels.
