Findings: Based on the Surveyors' observation, in the 3's and up room, there were four chairs stacked on top of a table, accessible to enrolled children. There were two mats (one on each side of the table) and a child was laying on a mat to the left of the table.

Findings: Per allegation, based on Surveyor #1's observation and staff interviews, a pre-school child was observed walking from the restroom to the 3's and up room alone and unsupervised. Staff stated the child really had to use the restroom so they sent the child to the restroom by themself. Staff stated the following: * We take the children to the restrooms in groups, but if a child "really has to go" we let them go alone. * Sometimes it happens that children go to the restroom alone, it usually happens around naptime. * Sometimes they just go to the restroom if they really have to go. * Sometimes it happens that we have to send children to the restroom alone, but not often. * Sometimes in the 3's and 4's they have to send children to the restroom alone if they have to go really bad.

Findings: Based on facility documentation and Surveyor #2's observations, the children were observed eating turkey sandwiches, potato salad, pineapple and milk. The menu stated that bread, beans, corn and milk would be served to enrolled children and these substitutions were not documented on the menu.

Findings: Based on the Surveyors' observations, in the 1's and 2's room, there was a strong odor of dirty diapers when the Surveyors entered the room.

Findings: Per allegation, based on the Surveyors' observations, and staff interview, in the school-age room, the tile floors were dirty with scuff marks and there were a few pieces of diced pears on the floor. Staff stated the children ate breakfast in that room earlier in the day and staff clean the room during naptime.

Findings: Based on facility documentation, the Surveyor's observations and staff interviews, the file for Staff #2 (DOE 7-8-14) lacked a valid fingerprint clearance card. The card in the file expired 7/23/2020. There was no new application in the file. Staff stated they were unaware the card had expired and no one informed them. The Licensee/director stated "I know she applied, but it's just not in the file".
