Noncompliance Area: Child #1 had an emergency contact form that lacked the addresses of the individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Staff person #1 had an initial health assessment on which the date of the health assessment had white out on the form. The date on the form was 9-23-19. The month and the year were written in one color ink and the year was written in a different color ink over top of the white out. It was unclear when the health assessment was administered.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: It was observed that the smoke alarm in the hallway outside of the upstairs bathroom was making a chirping noise causing the smoke alarm to be in disrepair. It was observed that the smoke alarm in the kitchenette in the upstairs of the facility was making a chirping noise causing the smoke alarm to be in disrepair. There were cords for a fan and a surveillance camera mounted high on the wall hanging down in to the childcare space making the cords accessible to the children and causing a possible hazard that the children could pull the items down.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Staff person #1, with a documented start date with children of 9-24-19, had an NSOR verification dated 9-1-20; which was obtained after the required time frame as designated by the CPSL.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Staff person #1 lacked an annual written evaluation.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: There was an electrical box on the floor in the rear room of the facility that had a cracked plate cover causing sharp edges to be accessible to the children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: There was damaged paint on the blue wall in the upstairs preschool room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: A mat in the rear room was observed to have a rip in the vinyl cover, causing foam to be exposed. A shelf on the bookshelf in the upstairs room was detached, causing sharp edges of a screw to protrude outward and be accessible to children. There were cracks on two plastic teeter-totters on the outdoor play area, causing pinch points to be accessible to children. There was a broken push toy on the outside play area, causing sharp plastic edges to be accessible to children. There was a brown climbing structure on the outdoor play area that was not properly assembled, causing the structure to be unstable. There was a plastic shelf on a bookshelf in the front room that was cracked, exposing the children to sharp edges and pinch points.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The file for Child #2 contained an immunization record that lacked a current flu vaccination, as recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The lid on the hands-free garbage can in the diapering area in the middle room downstairs was broken, causing it to not be hands-free.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: There was a plastic bag that contained a package of cigarettes and a disposable lighter on the floor, accessible to children, on the floor in the rear room downstairs.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: A spray bottle with a bleach and water solution that was not labeled or in its original container was observed in the middle room downstairs.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The refrigerator in the back room downstairs lacked a thermometer.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: Child #1, with an enrollment date of 5-18-18, had an initial health assessment dated 1-25-19.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: Child #3 had a previous health assessment dated 9-28-17 and a current health assessment dated 1-3-19.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.
