Noncompliance Area: DURING A RENEWAL INSEPCTION CERTIFICATION SUPERVISOR REVIEWED THE FILE OF STAFF #2; THE FILE DID NOT INCLUDE A STATE POLICE CLEARANCE AND MANDATED REPORTER TRAINING.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee.

Noncompliance Area: During the investigation the provider stated that she threated to hit child.

Correction Required: A facility person may not single out the child for ridicule, threaten harm to the child or the child's family and may not specifically aim to degrade the child or the child's family.

Noncompliance Area: During the investigation the provider confirmed that her husband and neighbor were interacting with the VC in attempting to deal with the VC's behaviors.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: Individual #1 (DOB-2/11/89) who resides in the home at least 30 days does not have proof of a state police clearance documented at the home.

Correction Required: At renewal, the legal entity shall submit clearances required by the CPSL for each individual 18 years of age or older who resides in the facility at least 30 days in a calendar year if the individual moved into the facility following the date of the previous application for a registration certificate.

Noncompliance Area: Child # 1 (enrolled 9/5/17) did not have a signed financial agreement with the services being provided documented.

Correction Required: An agreement shall be signed by the operator and the parent.An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: Child # 1 (enrolled 9/5/17) did not have physician information and health insurance information including policy number documented on their emergency contact form.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The financial agreement for child # 2 was not reviewed and/or updated within the last 6 months.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
