Noncompliance Area: 2nd citation: At approximately 8:45 am, staff person #1 stepped into the hallway and answered the front door, leaving a preschool child unsupervised in the kitchen, which is adjacent to the hallway. Approximately a few seconds later, staff person #1 left the kitchen again and walked down the hallway towards the back room to grab the touchless thermometer, leaving a preschool child unsupervised in the kitchen for several seconds, until the certification representative brought it to their attention. Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: There were several empty plastic bags in a small drawer of a plastic cart in the kitchen, making them accessible to the toddlers who were observed eating lunch in the kitchen.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: At approximately 9:30 am, staff person #2, the operator, walked down a short hallway and answered the front door, leaving 5 children alone in the main room, a combined group of 2 infants, 1 young toddler, 1 older toddler and a young school-ager.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: The last two child service reports in the file for child #4, a preschooler, were dated more than 6 months apart.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3280.124(f).

Noncompliance Area: The emergency contact form in the file for child #2 did not include any work information for the child's mother. The emergency contact form in the file for child #4 did not include any work information for the child's father.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.

Noncompliance Area: The emergency contact form in the file for child #2 did not include any health insurance information for the child. The emergency contact form in the file for child #3 did not include the policy number for the child's health insurance.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The file for child #4, enrolled since 2017, did not include any health reports for the child.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: The list of vaccinations in the file for child #1, #2, #3 and #5 did not include a flu shot or a written statement of exemption from a parent.

Correction Required: Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Noncompliance Area: Based on observation and a statement by staff person #2, the operator, they did not clean the diaper changing pad in between changing the diapers of two siblings.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Noncompliance Area: The last health assessment in the file for staff person #2 did not include an assessment of the person's suitability to provide child care.

Correction Required: An adult health assessment must include the physician's or CRNP's assessment of the person's suitability to provide child care.

Noncompliance Area: There was not a current copy of the liability insurance at the facility.

Correction Required: The legal entity shall have comprehensive general liability insurance to cover the persons who are on the premises. A current copy of the insurance policy shall be on file at the facility.

Noncompliance Area: Based on conversation with the operator, the facility does not have a written health and safety plan for COVID

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Correction Required: The facility's current certificate of compliance and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location.
