Our agency's health specialist will provide families with form who do not wish for their children to receive the flu vaccine. In the future, all parents will be required to provide all vaccine records. If the record does not show a flu vaccine, parents will be asked to complete a form stating that they do not wish to have their child receive a flu vaccine. |
