Plan: Director stated she will remind the teachers to let her know when they go out of ratio as she did not know the classroom was out of ratio. During time of visit, the director took some of the children out of the classroom in order to be within ratio. Director stated she walks around and checks the number of children in the classrooms about every 15 minutes.

Licensing specialist observed the 3 year old and older classroom to only have 1 teaching personnel to 16 children. Required ratio for that class should have been 1:15.

Plan: Director stated the dishes were not there last night so she will discuss with teachers that they are to take dirty dishes out of their classroom after each meal or feeding. In addition, the director will also walk through and check for dirty dishes or food everyday.

Licensing specialist observed the kitchen sink to have dirty dishes that appeared to be several days old. One container was dated 9/10/20 and a second container was dated 9/8/20. The container dated 9/8/20 had leftover food that was molded and had a smell to it.

Plan: Kristi went into the infant room to care for the children. Kristi had the staff member complete the training while licensing was present. Licensing worker viewed the safe sleep certificate. In the future, Kristi will make sure all safe sleep classes that are taken are approved. Kristi will only have staff members who have approved safe sleep training work in the infant room.

Plan: Kristi stated she will check on rooms that are not being used to make sure there is not food there. Kristi will check the facility for any food or anything else that has been left unclean everyday. Kristi will contact an exterminator today to come out in order to take care of the mouse issue.

Licensing worker observed mouse droppings in the drawers of the changing table in the 2 year old and older classroom. Licenisng worker also observed food sitting on the table in a classroom that was not being used that day that was molded and dated 7/28/20. In addition, there were food pieces (appeared to be chicken and rice) laying on the table and floor. There was also pieces of broccoli on the floor and table in the kitchen that director reported not to be from today as they had not served lunch yet.
