The shelf was unstable and tipped easily when light pressure was applied. Back yard patio area: * A bag of concrete was located on a dolly cart. * There was a large Fitness equipment set on the patio. * An Aloe plant was located on a low shelf. Back yard: * 2 shovels, several metal poles, plastic poles, and wood pieces were leaning against the shed. * Several tall pieces of raw, splintering wood were located in a waste container. Front yard: * There was chipped paint with exposed rough wood on the platform located under the bench swing. * A shovel was laying on the ground in the pea gravel area. * 2 large area carpets were bunched up in a pile on the gravel area near the fence. * The plastic border around the rubber chip area was moveable and not well-anchored to the ground.

The menu documented that milk, Kix cereal, and bananas were to be served for breakfast on the day of the inspection, however, the enrolled children were actually served milk, Cheerios cereal and mangos.
