Violation Observed: Ensure temperature throughout the center space is a minimum of 68 degrees, wherein two rooms were registering 63 and 66 degrees. | Note: 2/25/20-Unable to abate the violation as the temperature in the multipurpose room was 64 degrees and some children and staff were observed wearing jackets. The temperature was raised during the inspection, however, violation will remain open.

Space and room requirements are as follows: at no time shall a center allow more children in attendance than the licensed room capacity.

Violation Observed: Ensure center does not exceed it's licensed capacity, wherein center is licensed for 30 children and 32 children were present.

Environmental condition precautions are as follows: at the time of the initial application, any renewal application, relocation of an existing licensed center, and, as determined by the Office of Licensing, on a case-by-case basis, the facility operator shall submit to the Office of Licensing a Safe Building Interior Certification or other approval issued by the Department of Health that indicates that no further remediation is needed for the interior of the building in which the center is located.

Outdoor maintenance and sanitation requirements are as follows: centers that provide outdoor space shall maintain all fencing in proper condition.

Violation Observed: Ensure that the outdoor playground fencing is up and safely keeping the children inside the playground at all times, wherein the fence had been taken down by the church and the children were observed utilizing the playground without any fencing.

The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.

Violation Observed: Ensure all staff are aware how many children are present at all times, wherein staff reported 22 children present when 24 children were in attendance.
2019-04-08 3A:52-5.3(q)(1)(ii) Physical plant requirements for all centers
Space and room requirements are as follows: all space and rooms within the center to be used by children shall be inspected and approved by the Office of Licensing prior to their use. In making its determination, the Office of Licensing shall consider whether the space is too far removed, remote, or isolated from other areas of the center to be used by children. rooms or areas of the center that are not Office of Licensing-approved for use by children shall be made inaccessible to children.

Violation Observed: The center was observed using 3 classrooms and the church; all which are not on the center's licensed space. The center must submit paperwork to add the additional space to their license or cease use of the space.

Indoor maintenance and sanitation requirements are as follows: all televisions and computers shall be secured on a stable surface and shelving shall be secured and not be overloaded.
