Extended Day provides before and after-school childcare to families with K-6 children at nine of Glendale Elementary School District's seventeen schools. Our programs are located on the school sites, which means children are safe on campus and do not have to take a bus to an outside facility. We offer competitive rates, and a "drop-in" structure so that you are not paying for time that your child does not spend with us!

Initial Comments: The following deficiencies were observed at the annual compliance inspection conducted on 11-25-2025, and are subject to change pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the inspection. The DES group size checklist was compliant at the time of the inspection. 3 of 3 fingerprint clearance cards were verified to be valid via DPS during the inspection. The following items were discussed, but not limited to: 1. Outdoor activity area ramps and fall zones. 2. Procedures for using unlicensed spaces for emergencies. 3. Toilet room cleanliness/maintenance. 4. First aid kit components. 5. Staff annual training requirements. 6. Documentation of previous childcare experience in staff files. 7. Staff TB documentation requirements. 8. Updated forms available for download on the licensing website. 9. Emergency record documentation. 10. Storage of cleaning equipment. 11. Gym maintenance--monitor ceiling tiles. 12. New rules discussion (effective: 08-03-2025).

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 12/02/2024, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. The Empower Self-Evaluation link was emailed to the provider. The DES Group Size was observed in compliance at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Stock medications will be removed from First Aid kits *Monitor the cover plates in toilet rooms to ensure there are no gaps between the cover and the wall. *Monitor resilient surfacing to maintain six inches of loose sand in the required fall zones Compliance Officer is Heather Bauer.
