| Initial Comments: The following deficiencies were observed at the annual compliance inspection conducted on 11-25-2025, and are subject to change pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the inspection. The DES group size checklist was compliant at the time of the inspection. 3 of 3 fingerprint clearance cards were verified to be valid via DPS during the inspection. The following items were discussed, but not limited to: 1. Outdoor activity area ramps and fall zones. 2. Procedures for using unlicensed spaces for emergencies. 3. Toilet room cleanliness/maintenance. 4. First aid kit components. 5. Staff annual training requirements. 6. Documentation of previous childcare experience in staff files. 7. Staff TB documentation requirements. 8. Updated forms available for download on the licensing website. 9. Emergency record documentation. 10. Storage of cleaning equipment. 11. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection.
