Corrective Action: Maximum group size may not be exceeded and staff-to-child ratios must be maintained at all times. On March 30, March 31, and April 1, 2020, there were more than 12 children present with two staff members in a room with one-year-old children present. Toddlers and older children were combined for more than the first and last 90 minutes of the day on these three days. This was corrected on April 7, 2020, when the total number of children in the center dropped below 10.

Corrective Action: Children who exhibit signs of illness that need to be excluded from care should be kept in a separate area where they can be cared for until they can be removed from the center. A child was kept in a classroom for four hours with a fever and diarrhea until being picked up by their parent. Retrain staff members regarding excluding sick children from care with others by April 15, 2020.

Corrective Action: Toys that have been mouthed by children must be cleaned with soap and water and disinfected immediately. There was no evidence that this was occurring. This was corrected on April 7, 2020, when toddlers were no longer being cared for in the preschool classroom.

Corrective Action: Ensure there are no objects less than 1” in diameter that could create a choking hazard in a room with toddlers. Toddlers were being cared for in a preschool classroom where small objects, such as manipulatives, were present on the shelves. This was corrected on April 7, 2020, when toddlers were no longer being cared for in the preschool classroom.

Corrective Action: Ensure children are supervised at all times through the assignment of qualified staff members who are present and working with children. Video-footage showed that on August 23, 2019, an unqualified staff member was alone with 10 children, including four 1-year-olds, for approximately three minutes. Proper staff-to-child ratio was not maintained. Another staff member was observed alone with up to 13 children, including four 1-year-olds, on three occasions. Proper staff-to-child ratio and maximum group size were not maintained. Submit two weeks of attendance documentation for all classrooms.

Corrective Action: Ensure staff members who are providing care to children do not participate in personal activities. A staff member was observed on video using a cell phone while providing care to children. The staff member is no longer employed at the center. This concern was discussed at the time of the visit, and the center will review this requirement with all staff members.

Corrective Action: At the time of the visit, OCCL was notified that the administrator had resigned on September 16, 2019, and the curriculum coordinator was promoted to the role of Administrator. The center does not currently have a qualified curriculum coordinator. Ensure that OCCL is notified within five business days of the resignation, termination, transfer, or hiring of an early childhood administrator or curriculum coordinator. This was discussed at the time of the visit and center agreed to comply. Submit a plan showing how the HR functions will be handled by someone, other than the administrator, while she is serving dual roles.
