Hand sanitizer was stored on top of a microwave, which was on the counter top, not out of reach of the child as required.

A valid qualification letter was not on file and available to review at the facility. A copy of the qualification letter issued on 6-10-22 for a staff member with a hire date of 8-24-23 was not available for review.

A child was restrained as a form of discipline and the child's safety or the safety of others was not at risk. A four year old child was forced to remain in the small room in the back corner of the room. The staff member knelt in the doorway of the room and prevent the child from exiting the room with her body and arms. The staff member explained to the child that he was not allowed to leave the room based on his behavior.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet by the teacher's desk was not covered or in use.

Bleach and three cans of aerosol air freshener were stored in an unlocked cabinet in the bathroom, not in locked storage as required.
