In room 5, the free standing swiveling storage unit in the dramatic play area was unsteady due to 2 missing screws in the base and a protruding screw was observed on the unit's mirror, a broken blue storage bin and the legs of the yellow wooden chair were loose creating pinch points. In room 6, a tear was observed in a black bean bag chair causing the filling to fall out, 3 broken or cracked storage bins and a broken plastic faucet on the play sink were observed. In the bike room, the seat of a yellow floor scooter was cracked and broken.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Additionally, an extension cord was observed in use in the water room. The cord was threaded through a door jam with the door closed on the cord.
