Noncompliance Area: Observed Little Lambs Infant Room's first-aid kit was missing soap. Grasshoppers Room missing tape. Lucky Lions Room missing tape, tweezers and gloves. Tigers Room missing soap, an assortment of adhesive bandages, sterile gauze pads, tape, scissors and disposable, nonporous gloves. Alligators Room missing tape.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: Observed that Alligators Room first aid kit taken on excursions was missing a bottle of water. Lucky Lions Room first-aid kit taken on excursions missing tape, tweezers and gloves.

Correction Required: One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at 3270.75(c).

Noncompliance Area: Observed that bathrooms did not have hand washing sings posted stating a facility person and an able child shall wash his hands after toileting and before eating.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: Observed Little Lambs Room did not have evacuation routes posted.

Correction Required: Evacuation routes shall be posted.

Noncompliance Area: Observed infant asleep on bouncy chair. Staff person #3 was providing supervision.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child`s parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Staff person #1 knew 4 names out of the nine children he had assigned to him. Staff person #2 only knew 4 names out of the 11 children she was assigned to. Staff person #4 only knew 4 names out of the 10 children she was assigned to.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Staff person #1 grabbed child #1 by his arm and pulled him out of the classroom to the administrative office.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.
