Noncompliance Area: STAFF PERSON #1 WAS WORKING IN AN ADMINISTRATIVE POSITION AND DID NOT QUALIFY FOR THE POSITION FOR WHICH SHE WAS HIRED/ PERFORMING.

Correction Required: A group supervisor shall have an associate's degree from an accredited college or university, including 30 credit hours in early childhood education, child development, special education, elementary education or the human services field and 3 years of experience with children.

Noncompliance Area: ON 8/6/19 RENEWAL INSPECTION THE FACILITY LACKED (2) GROUP SUERVISORS NEEDED FOR THE MORE THAN 45 CHILDREN ENROLLED.

Correction Required: In a facility in which more than 45 children are enrolled, a group supervisor is required for the first group of 45 enrolled children and for each group or partial group of 45 additional enrolled children.Each additional group supervisor shall be employed by and be present at the facility for a minimum of 30 hours per week.

Noncompliance Area: PER THE ON-SITE COMPLAINT INVESTIGATION ON 7/19/19, THE ALLEGATIONS OF MICE AND MICE DROPPINGS THROUGHOUT THE FACILITY AND IN AREAS WHERE FOOD IS STORED AND/ OR PREPARED WAS VERIFIED BY THE STAFF AND DIRECTOR. THE INFESTATION PERIOD WAS FROM NOVEMBER 2018 THROUGH MARCH 2019 (APPROX).

Correction Required: Evidence of infestation of insects or rodents in the facility is not permitted. Food stored, prepared or served shall be clean, wholesome, free from spoilage, free from adulteration and safe for human consumption.

Noncompliance Area: At the time of the inspection the koala room had a soft slide and climbing blocks that were frayed and had exposed insulation rendering them in need of repair or replacement.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: At the time of the inspection the records of staff person #3 and #4 were lacking in current valid health assessments.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: At the time of the inspection the record of staff person #2 was lacking in a complete PA state criminal background check document.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: At the time of the inspection the record of staff person #1 was lacking in their current performance evaluation. Staff person #1 has served at the facility for over one year.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: Observed that the high chairs in the Little Lambs Infant Room did not have T-shaped safety straps.

Correction Required: High chairs shall have a wide base and a T-shaped safety strap.

Noncompliance Area: Observed that the Grasshoppers Room refrigerator did not contain a thermostat.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: Observed that Little Lambs Room, Lucky Lions Room, Tigers Room and Alligators Room did not have daily activities plan posted.

Correction Required: The written plan of daily activities shall be posted in the group space.
