Initial Comments: The following deficiencies were observed during the annual compliance inspection conducted on 4/20/2026, subject to changes pending programmatic review. Please submit the Plan of Corrections through the Licensing Portal within 10 days of receipt of the Statement of Deficiencies. Two of two Fingerprint Clearance Cards reviewed were valid through the DPS website. BCCL staff emailed the Empower Self-Evaluation Assessment link to the provider. The following was discussed, but not limited to: *Capacity is posted in all activity areas. *The first aid kit has all required components. *Staff files have all required information.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 4/22/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The Empower Self Assessment was emailed to the provider. The Emergency Disaster Contact Form was left with the provider on-site. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring that the good faith references are dated. 2). Ensuring food waste is in a garbage bin with a lid. 3). Ensuring that the bark chips on the playground are raked into the fall zones. 4). Ensuring the Emergency, Information, and Immunization cards are fully completed.

Initial Comments: The following deficiencies were observed at the time of the Compliance inspection conducted on 04/24/2024 subject to changes pending programmatic review. Compliance Officer (CO): Pat Morgan-Martinez Please submit the Written Documentation of Corrections via the Licensing portal within 10 days of receipt of the Statement of Deficiencies. 3 of 3 fingerprint clearance cards were valid via a DPS website search. The Empower Survey link was provided at the time of inspection. Please complete within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of inspection.
