Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports.

Noncompliance Area: At the time of the inspection in the 'Tigers' room more than half of the rest equipment has its plastic cover torn and tattered with stuffing coming out creating multiple choking hazards.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: At the time of the inspection while transitioning to the outdoor play space, one of the children in the chipmunks group was transporting the first aid kit.

Correction Required: A first-aid kit must be inaccessible to children.

Noncompliance Area: During the renewal inspection the certification representative viewed the children files. children #1, #2, #3, #4 #5 and #6 all needed updated emergency contact forms and financial agreement.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: During the renewal inspection the certification representative viewed the staff files staff # 3 missing the correct FBI clearance in the file.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 may not work in a childcare position at the facility.

Description: Refrigerator

Noncompliance Area: During the renewal inspection the certification representative conducted a facility walk through, the refrigerator in the infant room was missing a thermometer. The provider replaced the thermometer. The certification representative checked the temperature before leaving the facility and the thermostat was reading below 45 degrees F. citation was corrected onsite.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: During the renewal inspection the certification representative requested to view the emergency plan, the plans for continuity of care was not included in the plan.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: During the renewal inspection the certification representative conducted a facility walk through and observed a sippy cup in the Turtles room without a label.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: During the renewal inspection the certification representative viewed the emergency plan the plan did not state the method to contact parents. The plan to contact parents should be included in the plan.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises.

Description: Age and Training

Noncompliance Area: During the renewal inspection the certification representative viewed the staff files staff #4 was missing a CPR certificate in the file.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Noncompliance Area: During the complaint investigation the certification representative determined that child #1 did not have the updated immunization record in the file.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings).

Description: Building Surfaces

Noncompliance Area: During the renewal inspection conducted on 5/17/23, there was a divot in the outdoor play space beneath the the basketball hoop that posed as a tripping hazard for children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: Health and Safety Training - Pediatric First Aid and CPR

Noncompliance Area: At the time of the inspection the staff records indicated first aid completed by staff was not in compliance with regulatory requirements.

Correction Required: Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.
