Description: The implemented tracking procedure failed at the time of licensing visit. The attendance sheet is the tool the center uses for tracking purposes. The attendance/tracking forms did not accurately reflect the children present in all classrooms. 13 children were present in the infant room, five (5) were accounted for on the form. At lunch time, 23 children were present in the classroom, 22 were documented on the form. Children moved between rooms and areas without being tracked. A Notice of Order and Imposed Plan of Correction was issued November 28, 2018.

Description: Children were not within close supervision of a child care worker who was within sight and sound supervision to guide the childs behavior, activities, prevent harm and assure safety on numerous occasions during visit. Multiple children were observed in the school-age and preschool rooms without staff present. A child was observed at their cubby in the preschool room without staff present. A Warning Letter was issued July 7, 2017; a Notice of Order and Direct Forfeiture was issued October 22, 2018; a Notice of Order, Imposed Plan of Correction, and Forfeiture was issued November 28, 2018.

Description: The premise was not maintained clean and in good repair at the time of licensing visit. A hole was observed in the wall, above the baseboards in the middle room, baseboards were observed peeling away from the walls throughout the center, rust was observed on the front door frame, accessible to children.

Description: Indoor equipment was not of study construction or in good operating condition. A kitchenette in the infant room was not anchored, unit was easily tipped. A wooden bookcase in the school-age room was observed with sharp and broken edges. Multiple cots were observed broken in the school-age room, ripped with gaps.

Description: The mattress coverings observed on the Pack-N-Plays in the infant room were not tight fitting at the time of licensing visit.

Description: Two, open containers of leftover infant food were observed in the kitchen refrigerator, dated to have been opened March 28, 2019. Containers were not used or discarded within 36 hours.

Description: A child, under the age of one, was not placed to sleep on their back in a crib during licensing visit. Child was observed sleeping in a bouncy chair in the infant room upon arrival.

Description: Staff were not observed sitting with children during lunch time. Staff were observed moving between classrooms, and preparing for rest.

Description: A menu of meals and snacks for April was not posted at the time of licensing visit. The menus posted were from February - March 2019.
2019-04-02 251.05(4)(g) Mixed Age Group With Children Under Age 2 - Group Size
Description: Infants and toddlers were part of a mixed-age group during licensing visit, the group size exceeded eight (8). Upon arrival, 13 children were in the infant room, with five (5) being under the age of two.
2019-04-02 251.05(4)(f) Mixed-Age Group - Staff-To-Child Ratio
Description: The staff-to-child ratio was not adjusted on a prorata basis with a mixed age group. 23 children were present with one staff member, the prorata calculated to 2.031, meaning two staff were needed.

I have used this provider for more than 6 months

Great place for kids not only they beyond for your kids they teach them . And love em like their own. My 4 kids enjoy them

I have toured this provider's facility, but have not used its services

They violated ADA law. A report has been filed with the state. My son has ADHD and after seeing that "label", they called me after I turned in my paperwork saying they wont even take him. She never met my son. Shes never seen how he acts. How would she even know they cant accommodate? This was right after her saying "well, those must be crappy centers if they cant handle ghetto".
