Plan: immediately a supervision plan was put into place for both staff members responsible. The supervision plan is that anytime the staff involved are transitioning to or from the class, gym, or outside, admin staff must count kids to verify accuracy. Each staff involved was written up with a conference to discuss supervision. Write up was placed in their files and Each staff member involved was required to take "supervision: counting children" by December 16th. Documentation is in their files.

Plan: Cell phones are required to be “stowed” away in a drawer, inside a purse in a cabinet, or inside a locker in the break room. Cell phones are required to be out of sight. This also means they cannot be seen on your person. If you are seen with your phone not stowed away: • Write up and your cell phone can not be in classroom-only locker. Second time-Write up and cell phone is not allowed on premises.
