Violation Observed: Based on information, ensure that adequate supervision is maintained at all times throughout the center. A child was observed unsupervised by the front desk. Additionally, a staff was observed on her phone while caring for the children. On 3/1/23 staff needed to step out of the classroom or answer the door leaving the children unsupervised.
