Noncompliance Area: A handwashing sign was not posted at one of the toilets in the boys bathroom on the 1st floor and at one of the sinks in the boys and girls bathrooms on the 2nd floor.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: According to staff statements and documentation including pictures, it was determined that staff person #1 pulled child #1 out from under a table by their legs resulting in brush burns/red marks on the child's back. The incident was self-reported by the operator.

Noncompliance Area: Several pairs of Colorations brand, blue blunt tip scissors were observed to be accessible to preschoolers in the Fellowship Hall. Based on the manufacturer's description, the scissors are intended for ages 5+ and grades KG+.

Correction Required: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use.

Noncompliance Area: A pinch point was observed on a child size gray chair in the downstairs preschool room. The back rest of the chair was not secured tightly to the frame.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

When asked to name their primary care groups , staff #3 named 2 children, staff #6 named 5 children, staff #12 named 4 children, and Staff #13 named 3 children. Staff in the room named 14 children leaving 1 child, child #5, unnamed. Staff #5 was assigned to child #5, but staff #5 was out of the room assisting another child in a restroom.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Correction Required: When the number of older toddler, preschool or school-age children participating in a program activity exceeds the capacity of the child care space each time period shall be designated on the facility's schedule of daily activities.
