|Findings: The Team Leader reviewed 6 Emergency, Information, and Immunization Record cards and found the following: 1. Child #1 was missing the date of enrollment. 2. Child #2 was missing the health care provider's phone number.|
|Findings: The Team Leader observed that the immunization record for Child #1 was not current. Child #1 is a year old and only has the first series of shots.|
|Findings: The Surveyor reviewed the sign in and out records and found the following: 1. Child #1, #3 and #4 were not signed out on 7/2/21. 2. Child #5 was not signed out on 7/1/21.|
|Findings: The Team Leader observed a medication for Child #6 that was expired.|
|Findings: The Surveyor observed the following hazards in the outdoor area: 1. A metal hinge with sharp prongs accessible to children. 2. A rubber tub filled with water. Pictures were taken of the hazards.|
|Findings: The Surveyor observed a bottle with formula in the crib. The Provider stated they did not have written instructions by the parent.|
|Findings: The Surveyor observed that children were eating food that was not on the menu. The substitutions were not noted on the menu.|
|Findings: The Surveyor observed 2 bottles that were not labeled with an identifier.|
|Findings: The Team Leader observed that there was no documentation of fire drills. The Provider stated they did a reminder on their phone for fire drills.|
|Findings: The Surveyor observed that the paper towels were sitting on the sink and were not in a dispenser.|
|Findings: The Surveyor was not granted immediate access to the group home. Staff 1 answered the door and told Surveyor that the Provider was not there. The Surveyor tried to enter and Staff 1 said they had to call the Provider first and shut the door. The Surveyor was left outside and not granted access from 3:05 pm to 3:11 pm. After 6 minutes, Staff 1 opened the door to allow the Surveyor inside.|
|Findings: Upon arrival at the group home, Staff 1 was acting on behalf of the provider. Staff 1 does not have a high school diploma or high school equivalency certificate.|
|Findings: Upon arrival at the group home, the Surveyor was told by Staff 1 that the provider was not there. The Surveyor observed that Staff 1 was alone with 6 enrolled children. Previously cited on 1/14/21.|
|Findings: The Provider told the Surveyor that Staff 1 did not have current first aid and CPR. Staff 1 was alone with enrolled children upon arrival at the group home. Previously cited on 1/14/21.|
