In the future all new hire staff including all employees will have TB test / physical before start date; and be required to submit record of their TB test results including their physical to be on file. Correction was made, staff person #1 scheduled their appointment but staff #1 never showed up or returned back to work. Staff #1 is no longer employed with us here at Bright Minds.

In the future all new hire staff and all employees will have to have verified employment - status instead of volunteer on their child abuse clearance before starting employment with us at Bright Minds. Correction was made: Staff person #1 went the next day to get her child abuse clearance and it was up-dated to employment and placed in her file. Staff person #1 is no longer employed with us. Documentation of updated clearance was faxed to Sara on July 7, 2018.

The small chair with the section of peeling paint on the armrest was corrected on site by placing duct tape on the area that was peeling. In the future all paint will be taken care of and all chairs will be check daily to see if need painted.
