Noncompliance Area: During naptime in the preschool classroom, it was observed that the music playing in the room was set at an excessively high volume which prevented proper supervision (see, hear, direct and assess) of the children while napping.

Observed the threshold/foot plate of the door leading outside from the multipurpose room was loose and slides out of place.

Noncompliance Area: Observed child #3, a preschooler (see LIS code sheet), resting on a cot intended for a toddler (ages 12 mos. - 36 months). While lying stretched out on the cot, the child's feet were hanging over foot of the cot due to the length being too small. Staff person #4 removed the child's diaper and placed it on the diaper changing station above the child's hand instead of immediately discarding the diaper into the plastic-lined, hands-free covered can next to the changing table.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: Observed staff person #1 send 2 toddlers to the table in the classroom for snack without ensuring that their hands were washed before serving them snack. Noncompliance Area: Observed the facility had 1 bottle in the refrigerator in the toddler classroom for child #1. When asked if there were additional bottles for the child staff person #3 stated that if needed, they would wash the bottle in the sink so it can be reused. The facility does not have a commercial dishwasher to wash bottles, and the parent only supplies them with one bottle. The feeding schedule provided by the parent indicates the child should be fed 4 times/day and would need to provide 4 bottles for use. Continued noncompliance previously cited on 01/16/2026. "Observed the facility had 1 bottle in the toddler classroom for child #1. When asked how the child would be given additional feedings, staff person #2 stated that the bottle will be washed in the sink and reused. The facility does not have a commercial dishwasher to wash bottles."

Description: Mixed Age Level

Noncompliance Area: Upon arrival at approximately 2:15 pm, observed staff person #3 & #4 in the preschool classroom during naptime. During this time, staff person #1 was working alone in the toddler classroom with 1 infant, 1 young toddler, 2 older toddlers and 4 preschool children. Staff person #1 stated they had been alone for approximately 1/2 hour while staff person #3 was taking their break. Staff:child ratios for infants is 1:4. Naptime ratios do not apply to infants.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff: child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level).

Noncompliance Area: Observed the cover was open on a section of the baseboard heat in the preschool classroom and the school age classroom; exposing the heat elements within which reached a temperature exceeding 110° F and were accessible to children.

Observed the children playing in the preschool classroom were limited to 2 activities at the tables and the children were not permitted to choose activities based on individual interests.

If the running water does not flow directly into a drain that is connected to a sewage system, a receptacle shall be provided to contain the water used for washing. The receptacle shall be emptied into an approved sewage system at least once a day.

Description: Promote development

Noncompliance Area: Observed a group of 7 older toddlers and 1 preschooler in the toddler classroom. The children were not engaged in play or other activity upon arrival to the classroom. The TV was on and staff were sending the children to the area in front of the TV to watch a program in lieu of age-appropriate activities or play. The children were then brought to the table to color a picture while the TV remained on in the background. Continued noncompliance previously cited on 07/23/2025, Observed 2 young toddlers and 3 older toddlers in the toddler classroom sitting at the table coloring and engaged in crafts for an extended period of time. Staff were not actively directing the activity for the development of skills and were unable to maintain the toddler's attention. The TV was on at a high volume and caused distraction for the children at the table. The activities were not age appropriate and the children were sitting at the table for a period of at least 1 hour.

Correction Required: Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.

Description: Infant/Toddler Stimulation

Noncompliance Area: Observed staff person #3 and #4 were not actively engaged with the group of older toddlers and preschoolers in the room. Staff person #4 was preparing a coloring page while the children were in front of the TV. Staff person #3 was not engaged with the children by talking to them, playing with them or interacting with them while staff person #4 was busy. Continued noncompliance previously cited on 07/23/2025, Observed staff person #1 and #2 were not actively engaged in conversation with the children in the toddler classroom during table activities. Provider failing to follow plans to maintain compliance regarding same violations from inspection(s) of 07/23/2025, 05/15/2025 and 04/17/2025. Description: No physical punishment

Noncompliance Area: Observed staff person #3 grabbed a child who did not want to go to watch the program on the TV. The child dropped themself to the floor and staff person #3 picked the child up around the child's body and attempted to move them. Staff person #3 was informed by Certification Staff to let the child go and they did not immediately do so, they continued to attempt to move the child toward the TV. Once the child was let go, the child was running around the table instead of going to watch the TV. The child was not causing harm to himself or to any other children in the classroom.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Staff person #4 was in the facility but was not permitted to be in ratio with the children due to an expired NSOR clearance.

Observed the mat below the water fountain located in the hallway with caked on dirt and the water fountain with visible stains. Continued noncompliance previously cited on 11/22/2024. On 01/30/2025, observed a fluorescent light with a light cover which was broken and popping off the light and 5 out of 18 lights which were not working in the multipurpose room downstairs. Observed the exit door to the playground and the baseboards in the multipurpose room had visible dirt. Continued noncompliance previously cited on 10/24/2024 and 11/22/2024, On 01/30/2025, observed the baseboard trim, windows and windowsills throughout the building were visibly dirty with dust and dirt.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: At 9:40 am, observed child #2, an older toddler sleeping in a highchair in the preschool classroom. According to staff, the child had been sleeping since approximately 9:05 am. Staff did not move the child onto age appropriate rest equipment during this time.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The written plan of daily activities and routines do not include opportunities for staff directed activities and interactions. Children are observed engaged in free play throughout the day while staff are sitting and watching them play in lieu of interacting with the children in staff lead learning and development activities. Staff led activities listed in the daily schedule which are not being implemented include circle time/story time, fine motor play, music & movement, and art activities. The daily activities do not include opportunities for age-appropriate learning to develop skills, social competence and self-esteem.

Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group. Daily activities shall promote the development of skills, social competence and self-esteem. Daily experiences shall recognize the child as an individual and give some choice of activities that respect personal privacy, lifestyle and cultural background.

Noncompliance Area: On 11/27/2024, staff person #1 was engaged in a personal video call with an inmate on their cell phone while caring for children in the preschool classroom. During this time, staff person #1 was not properly supervising the children.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Staff person #1 stated the milk was brought upstairs for breakfast at approximately 7:00 am. At approximately 9:40 am, the milk was returned to the refrigerator after sitting at room temperature for approximately 2 1/2 hours.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Correction Required: A facility person may not disclose information concerning a child or family, except in the course of inspections and investigations by agents of the Department.

Noncompliance Area: At lunch, observed staff person #3 serve peaches to child #1. At that time, child #1 stated that they can't have peaches, they are allergic to them. Staff $2 and #3 were unaware that the child had an allergy to peaches and thought they were allergic to blueberries. Upon review of the emergency contact and health report for child #1, peaches were listed by both the parent and the child's physician that the child is allergic to peaches and to administer an epi-pen if injested. An epi-pen was not onsite at the facility.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. The operator shall make reasonable accommodation in accordance with applicable federal and state laws to facilitate administration of medication or a special diet that is prescribed by a physician, physician's assistant or CRNP as treatment related to the child's special needs. Facility persons are not required to administer medication or special diets which are requested or required by a parent, a physician, a physician's assistant or a CRNP but are not treatment related to the child's special needs.

Noncompliance Area: Observed staff person #1 leave the preschool classroom to return breakfast items to the kitchen, during this time staff person #2 was observed caring for a mixed group of 2 older toddlers and 8 preschool age children. The staff to child ratio when an older toddler in in care is 1:6.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level).
