Noncompliance Area: Child #1 was observed having an enrollment date of 9/13/16 and initial health report dated 12/19/19, exceeding 60 days after enrollment.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: Staff #1 was observed having Mandated Reporter training completed by a provider which is not accepted by DHS for childcare.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: The main child care space located in the basement was observed having a steel beam in the middle of the play space which lacked any form of cushion creating a hazard to children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: The doorway leading from the kitchen to the basement was observed opening directly into the stairwell. The doorway lacked any restriction from opening to prevent children access to the stairwell while lunch is served in the kitchen.

Correction Required: If a door or doorway opens or exits directly into a stairwell and if there is no landing beyond the door or doorway, the door shall be restricted from opening or shall be removed and a secure barrier to prevent access to the stairwell shall be erected.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The files for children #2 (enrollment date August 2014) and #4 (enrollment date in March 2015) have a recent child service report completed on 02/20/2017, the file for child #3 (enrollment date in March 2014) has a recent child service report completed on 01/17/2017 with no prior child service reports on file. The file for child #6 (enrollment date in August 2016) has a child service report dated 04/11/2017 with no prior child service reports on file.

Noncompliance Area: Observed shampoo in the shower located in the bathroom used by children in care which were accessible to children. Observed carpet fresheners in an unlocked closet which were accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The first-aid kit lacked soap. The facility lacked a first-aid kit that accompanies staff and facility persons on walking excursions from the facility.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at 3290.73(c).

Noncompliance Area: Observed a 6 ft. tall book case in the indoor child care space which was not anchored creating a tip hazard. Observed an unlocked door leading to the exercise room with weight equipment which was accessible to the children in care. Observed a gas grill on the porch located in the outdoor play space which lacked a cover and was accessible to children. Observed a hobby horse in the outdoor play space with a crack in the base. Observed holes in the Little Tykes log cabin where the shutters were removed creating rough edges. Observed plastic bins on a shelf in the indoor play space which were cracked creating a pinch hazard and rough edges.
