Noncompliance Area: It was observed in the upstairs bathroom an unlocked cupboard containing shaving cream, bleach water, Drano, Clorox Clean Up and Poison Ivy cream accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: It was observed in the toddler bathroom that drywall by the sink area was crumbling and falling apart.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: Toxic plants not permitted

Noncompliance Area: It was observed in the outdoor play area many toxic weeds and a large bush.

Correction Required: Toxic plants are not permitted in a child care space.

Regulation: 3270.77(a)

Description: No peeling paint or plaster

Noncompliance Area: It was observed on the outside building of the playground area that peeling paint was falling off and chips were on the ground accessible to children.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: 2nd CITATION, On 8/24/20 at approximately 1430, 1 infant was observed sleeping in a bouncy seat and 1 infant was observed sleeping in a infant swing, which is non compliant with the infant sleeping positions recommended by the American Academy of Pediatrics. PREVIOUSLY CITED AT RENEWAL INSPECTION ON 8/6/20.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: A children's cubby which measured approximately 4 feet in height and 4 feet in width located in the school-age child care space lacked anchors to the wall creating a possible tipping hazard.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The infant/young toddler child care space was observed having an infant sleeping in a children's swing.

Noncompliance Area: The outdoor play space was observed having a children's play house with broken plastic at the doorway creating a sharp corner.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The toddler room was observed having 2 plastic cabinets with an approximate 6 foot height and 3 foot width which lacked anchoring to the wall creating a possible tipping hazard.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The emergency contact information for child #1 lacked the address of the individual designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The bathroom located in the basement toddler child care space lacked a hands-free covered can for the immediate disposal of diapers as a result of the hands-free mechanism in the covered can not working correctly.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: The staircase leading to the outdoor play space from the basement kitchen lacked adequate lighting as a result of the light bulb above the door being burnt out.

Correction Required: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light.

Noncompliance Area: According to video documentation, on 10/15/18, staff #1 was observed pushing child #1's face off of her arm while the child was biting her. As a result of pushing the child's face off her arm, child #1 hit their head against the wall, causing child #1 to cry.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Noncompliance Area: According to video documentation, on 10/15/18, staff #1 was observed pushing child #1's face off of her arm while the child was biting her. As a result of pushing the child's face off her arm, child #1 hit their head against the wall, causing child #1 to cry. On 10/15/18 the parent was not made aware of the incident. On 10/19/18 the parent was informed about the incident by an outside resource. The parent then discussed the incident with the facility on 10/19/18, 4 days after the incident had occurred and provide no written documentation.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Noncompliance Area: The 1st floor school-age room was observed having a green cushion which was torn in multiple places. The infant room was observed having a changing pad that was torn with exposed foam.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The foyer area located at the entrance of the facility was observed having two cabinets which lacked anchoring to the wall creating a possible tipping hazard.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The files for children #1 and #2 lacked the Department's approved form (Child Service Report) to provide information to the family about the child's growth and development in the context of the services being provided.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The current health assessment dated 8/15/17 for staff #6 lacked documentation of an examination for communicable diseases and the results of that examination.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination.

Noncompliance Area: The file for child #1 lacked an agreement updated at least once in a 6-month period. The last update observed was dated 11/5/17.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The files for staff #1, #2, #3, and #6 lacked training regarding the emergency plan at the time of initial employment.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: The file for staff #2 lacked a copy of their Mandated Reporter training present in the file. The file for staff #3 was observed having a hire date of 6/11/18 and an updated FBI clearance dated 6/16/18, but lacked the previous FBI clearance utilized upon hire present in the file. The file for staff #6 was observed having a volunteer Pennsylvania Child Abuse clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Staff #6 may not work in a child care position at the facility until a complete clearance is obtained. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff #6 may not work in a child care position at the facility until a complete clearance is obtained.

Noncompliance Area: The file for staff #5 lacked a staff evaluation completed every 12 months.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: During lunch time children were observed entering the lunch room to be seated at the table while the stove was observed having a pot of boiling pasta on the front burner accessible to children.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F. Hot water pipes and other sources of heat exceeding 110 F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Correction Required: Disposable nursers shall be used for infants unless bottles are provided by the parent or unless a commercial dishwasher is used by the facility.

Noncompliance Area: The infant room was observed having a crock pot for warming bottles sitting on a doilie located on top of a microwave. The microwave was observed having a children's plastic chair beside it making the doilie within a child's reach and enabling the crock pot to be pulled off the microwave.

Noncompliance Area: Peeling paint was observed on the back wall at the bottom of the staircase leading to the entrance of the toddler and infant rooms. Peeling paint was also observed on the block foundation in the outdoor playspace.
