The staff were instructed to only place the infants to sleep in cribs and beds. The swings and seats are for entertainment. They have been using timers to keep track of how long a baby is in a seat to make sure they don't sleep in a swing or bouncy seat, and move them to a bed for sleeping.

The director and staff were informed that all incidents need to be reported the day of the event. The parent needs to be informed. Each classroom teacher has forms in their room and will fill them out, have the parents sign them and then submit to the office. If staff do not feel comfortable the director or owner will inform the parent. We had two trainings, One on supervision and another on how to handle difficult children. All training was completed on 2/25/19.

All children's files will now use the DHS child service report instead of our current evaluations that we use. All classrooms were given the approved form to update all files. Child #2 is no longer enrolled. Child service report was completed for child #1.
