Noncompliance Area: A renewal inspection was conducted on 11/6/25. Cert rep observed the Preschool classroom to be over capacity and this was not indicated on the facility's schedule of daily activities for a 30-minute special activity. The Preschool classroom had 17 children and the capacity of the classroom is 14 children.

Correction Required: The capacity established for indoor space may not be exceeded except when older toddler, preschool or school-age children are participating in a program activity. When the number of older toddler, preschool or school-age children participating in a program activity exceeds the capacity of the child care space each time period shall be designated on the facility's schedule of daily activities.

Noncompliance Area: A renewal inspection was conducted on 11/6/25. The first aid kit in the Toddler classroom did not contain disposable, nonporous gloves. In the Infant/Wobblers classroom there was a bottle of cleaning solution that did not specify the contents and was not labeled.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces. A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: A renewal inspection was conducted 2/7/25-2/10/25. The facility's parent handbook did not contain a supervision policy.

Correction Required: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.

Noncompliance Area: A renewal inspection was conducted 2/7/25-2/10/25. Child #2's initial health assessment was not received within 60 days following the first day of attendance at the facility. Child #2's initial health assessment is dated 11/28/23. (See code sheet for date of admissions).

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Description: Immunization record/Dismissal policy

Noncompliance Area: A renewal inspection was conducted 2/7/25-2/10/25. Child #3's immunization record indicates the child does not have the MMR or Varicella vaccine and only one dose of the Hepatitis A vaccine. There is no exemption letter is in child's file. This is not in accordance with the ACIP recommended schedule.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in childcare group settings). Child #3 will be removed from care by close of business on 2/7/25 until either the vaccination record is updated according to the ACIP recommended schedule or the parent provides an exemption letter.

Noncompliance Area: A renewal inspection was conducted 2/7/25-2/10/25. Cert rep observed in the toddler's classroom Nystatin cream marked prescription on the box. There was no RX label to indicate the name of the child and the box didn't contain instructions from the prescriber. Staff was unable to say who the medication belonged to or if the medication was administered.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: A renewal inspection was conducted 2/7/25-2/10/25. Staff person #1 has completed the health and safety course titled Get Started with Center-Based Care: Building Blocks for Quality but has not completed the required health and safety update course titled Pennsylvania Health and Safety Update 2022 (1 hour) training. The 1-hour update was required to completed by 12/30/22.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Staff person #1 must complete PA Required Health and Safety for Centers-Group Child Care Parts 1 and 2.

Noncompliance Area: A renewal inspection was conducted 2/7/25-2/10/25. The facility did not have documentation on file that the emergency plan was sent to the local municipality and to the county emergency management agency. The facility did not have documentation that the emergency plan was reviewed annually.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan and kept on file at the facility. The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency.

Noncompliance Area: A renewal inspection was conducted 2/14/24. At that time, small plastic bags were observed in a diaper bag located at floor level of the cubbies in the Wobbler Room, where they were accessible to the young toddlers.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: A renewal inspection was conducted 2/14/24. At that time, Children's Tylenol observed in a diaper bag located at floor level of the cubbies in the Wobbler Room, where they were accessible to the young toddlers.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.
