Staff were occasionally signing into specific classrooms but were failing to sign out. Staff did not sign in at all in several instances.|

Staff indicated they previously attempted to use cones to keep the group separate; however, staff stated children picked them up and played with them. When Staff C was asked which children were assigned to her, she identified six children who were intermixed with the other children in the group.|

|Description: Based on a review of attendance records and staff interviews, it was determined staff-to-child ratios were not maintained on several occasions from January 2 to February 8. In the Infant Room, there were days where one staff was left supervising 6 children for 15-30 minutes. In the Big Tots Room, one staff was left supervising 13 children, 2 1/2 to 3 years. On 2/15/19, 2/19/19, 2/20/19, one staff was left supervising eight children under 2 years old for several hours in Toddler 2. On 3/7/19, in Toddler 1, Staff D was left supervising 8 children under 2 years old.|
|Description: Routines between activities and meal times are not planned to avoid children waiting in lines. On 02/11/2019, in the Big Tots Room, children were seated at the tables and had to wait approximately 10 minutes while breakfast was plated prior to being served their meal. On 03/12/2019, in the Big Tots Room, some children waited approximately 20 minutes from the time they started getting ready until the time they went outside. Upon returning to the classroom, children then had to wait 10 minutes from the time they sat down for lunch until it was plated and they were allowed to select a plate.|
